Case Management

Monday-Friday from 9:00am-5:00pm

HomewardBound has a case manager available for resource navigation, housing resources and other services for guests. This service will be taking place at the HomewardBound Navigation Center at 2853 North Avenue. Please call 970-256-9424 for more information.

The HomewardBound case manager helps guests with a variety of items including:

1. Referrals to various community agencies for their expertise. The unhoused situation is far too vast for any one agency to take on themselves.
2. Doing a vulnerability index assessment (VI-SPDAT) to evaluate the needs.
3. Adding folks to housing voucher waitlists if applicable.
4. Help find employment, work on resumes, interview skills. Collaborates with the Workforce center. 
5. Help enroll or check on Medicaid, SNAP, ect.

Service include basic first aid, health education, disease education/management, clarification of an established medical diagnosis, referral assistance for additional medical care, vital readings and limited over-the-counter medications. This service will be taking place at HomewardBound Navigation Center, 2853 North Avenue. Please call 970-256-9424 for more information.

MAP Food Distribution Service

Weekly on Tuesday from 11:00am-1:00pm
Mutual Aid Partners (MAP) provides a weekly, free community choice food pantry, hygenie essentials, free haircuts, resource navigation and other services. Open to all members of the community, choice based allows for guests to access items individualized to them. This service will be taking place beginning 6/17/25 at 402 Grand Avenue in the parking lot.
