Case Management

Daily from 9:00am-5:00pm

HomewardBound has a case manager available for resource navigation, housing resources and other services for guests at the RC to access.

The HomewardBound case manager helps guests with a variety of items including:

1. Referrals to various community agencies for their expertise. The unhoused situation is far too vast for any one agency to take on themselves.
2. Doing a vulnerability index assessment (VI-SPDAT) to evaluate the needs.
3. Adding folks to housing voucher waitlists if applicable.
4. Help find employment, work on resumes, interview skills. Collaborates with the Workforce center. 
5. Help enroll or check on Medicaid, SNAP, ect.

Service include basic first aid, health education, disease education/management, clarification of an established medical diagnosis, referral assistance for additional medical care, vital readings and limited over-the-counter medications.
